Care Plan Service Includes:
Personal Consults
Online video consultations as needed to discuss your health needs – including prescriptions, requisitions, referrals.
Unlimited messaging
Unlimited access to message NP Katrina about your health (please allow up to 3 days for response over weekends/holidays)
Continuity of Care
Shared care with your primary provider through consult notes and collaboration to reduce outside use penalties.
Notes / Forms
Single page accomodation notes + $20
Multiple page medical forms + $200
